Black Belt Requirements:

1. Complete app form, promotional exam record, black belt database form - need those forms before I can proceed (LATE JULY)

2. 2 passport photos, + 2 photocopies - JUNE

3. Promotional fees - AUGUST

4. 5 page book report - JULY

5. 12 page research paper - JUNE

6. CD of 4 & 5 - AUGUST

7. Professional Resume - JUNE

8. Martial Arts/Volunteer Resume - JUNE

9. CPR/First Aid - 3 photocopies - JULY (after July 17 CPR recertification)

10. Training Calendar - AUGUST (Should put this up ASAP anyway so that Laura/Lisa can check)
